• Personal Profile including Driver`s License or Passport Number.
• Annual Health Plan deductible and date coverage begins.
• Dependents (for family plans).
• Bank Account information including Routing and Account Number.

• Include a check for the one-time set up fee, monthly administration fee and initial deposit to the HSA account. A $100 minimum opening deposit is required to establish your account.
